PERSONAL ASSISTANCE
This program is designed to promote independence for a developmentally disabled adult (18 years and older), at home and in the community. The assistant works with the individual to help him or her learn life skills through modeling and training in the individuals' natural setting.
DAY HABILITATION SERVICES
Day habilitation services are provided to promote inclusion and self-determination. These services are based on personal goals set in each individual's support plan. Services may include participation in community activities, learning daily living skills, promoting positive social interactions and any other activities designed to accommodate his or her skill level. These services can be provided in a group setting or to an individual either on-site at our Ability center or in the community.
JOHN H. STRATMAN APARTMENTS
The John H. Stratman Apartments give consumers the opportunity to live independently in a safe, appropriate, and affordable house setting. The guidelines and qualifications are set by the Department of Housing and Urban Development (HUD). Independent living services allow consumers to have the freedom to choose their own place to live along with supports with cooking, cleaning, personal care, budgeting, and other activities of daily living. Consumers may also receive supports such as adaptive equipment, medical transportation, and health and/or medical assistance. Social activities are encouraged and community based. Some examples might be attending local community picnics, bowling, and other planned trips in the area.
